At our clinic, we obtain your consent to collect test data and related information. In this notice, for convenience, we collectively refer to individuals who receive services at our clinic (treatments, procedures, health checkups, etc.) as “patients.”
Our clinic will jointly use personal information as described below. If you do not agree with the content of this notice, please inform the reception desk. If we do not receive a request from you, we will treat it as your consent.
1. Items to be collected and jointly used
- Test data, a portion of specimens (blood, tissue, etc.), and medical images such as CT and MRI (hereinafter referred to as “specimens, etc.”).
2. Purpose of joint use and how the information will be used
- Data related to specimens, etc. will be stored at our clinic and related institutions (Elixion Co., Ltd. (Data Management Officer: Kazuhiro Hiyama; hereinafter referred to as “medical institutions”)) and used for research, education, and quality control for testing operations. Specimens, etc. will be analyzed and statistical analyses will be conducted.
3. Intellectual property rights and compliance with guidelines
- When using specimens, etc. for research, we will comply with Japan’s “Ethical Guidelines for Medical and Biological Research Involving Human Subjects.”
- Intellectual property rights arising from research outcomes shall belong to our clinic and the related institutions.
4. Protection of privacy and storage
- Specimens, etc. will be appropriately stored and retained by our clinic and related institutions in accordance with the Act on the Protection of Personal Information and related regulations. Data obtained as a result of the use of specimens, etc. will be retained for three years after use. After the retention period ends, such data will be promptly disposed of and deleted in an irrecoverable manner.
- In using specimens, etc., we will give full consideration to the Act on the Protection of Personal Information. All information that could identify an individual will be replaced with a research ID, and the data will be anonymized so that individuals cannot be identified.
5. Freedom to consent and withdrawal (opt-out)
- Even if you do not agree with this content, you will not suffer any disadvantage in medical care.
- Even after consent is deemed to have been given once, you may withdraw your consent (request suspension of use) at any time.
- Upon receiving a request for withdrawal, we will take appropriate measures such as discontinuing the storage of specimens and data and promptly disposing of them.
